Welcome to the Skaggs Sleep Disorders Center. We have links to forms below to increase your ease of ordering Sleep Studies for your patients. Please take a moment to read through the following directions, and feel free to call the center at (417) 335-7558 if you have any further questions/ concerns or when you are ready to schedule you patient for sleep testing.
Direct Referral
As an Accredited Sleep Disorders Center we offer the primary care physician the ability to directly refer your patients into the Sleep Disorders Center.
A direct referral is a patient who is never seen in consultation by a sleep staff physician. The referring physician (PCP for purposes of this document although it is recognized physicians other than primary care providers may send patients as direct referrals) orders the sleep study, then treats and provides longitudinal care for this patient. The studies are interpreted by our AASM diplomate and the report is sent directly back to the ordering physician. The ordering physician is responsible for the initial H&P, insurance pre certification (the necessary CPT codes are on the order form next to the procedures), prior to testing, ordering of mechanical (PAP) and/or pharmaceutical therapies if indicated, and follow up.

Sleep Consult
We will also offer a full sleep workup by our Board Certified Sleep Physician, who will be responsible for the longitudinal care of your patient while undergoing all aspects of testing in the Sleep Center.
